PDF Files: Download, print and fill out by hand. Scan and email back to me at kristi@innatewholenesshealing.com at least two hours before your appointment. If you don't have a scanner but have an iPhone you can scan documents and send by email from the Notes app. Click the camera icon and you will be given the option to Scan.

Word Docs: This is a good option if you don't have a printer. You can download and open the documents to fill out on your computer then email back to me, again at least two hours before your appointment.
